There should be a certificate at the end that you print out and staple to the back of the above application form in order to prove that you took the virtual course above.

Safety Training History

4/7/12

Dr. Forest organized a safety training session in the Physics Machine Shop by machinist Kenneth Moore at 2 pm. Four graduate students and Five undergraduate students participated. Dr. McNulty also participated. A Video was shown in the Physics conference room and then Kenneth Moore took participants down to the Physics machine shop for machine specific safety training.
